Can someone please explain this part to me:

 Note that for a given altitude triangle ABC, there can be four possible triangles DEF
What I was thinking was: Find O, Draw lines orthogonal to OV at V (V={A,B,C}), there will be 3 intersection points, that's our triangle DEF.

But that is obviously wrong, because that way (I think) I get only one triangle?
